Conditions: 100 - No supply of alcohol may be made under the Premises Licence - (a). At a time when there is no Designated Premises Supervisor in respect of the Premises Licence; or (b). At a time when the Designated Premises Supervisor does not hold a Personal Licence or his Personal Licence is suspended.

101 - Every supply of alcohol under the Premises Licence must be made, or authorised by, a person who holds a Personal Licence.

107 - Any individual carrying out security activities at the premises must be licensed by the Security Industry Authority. This does not apply where the premises are being used primarily as a Qualifying Club Under a Club Premises Certificate, under a Temporary Event Notice, or primarily as a cinema, restaurant or theatre.

111 - This licence provides for the provision of private music and dancing entertainment that is promoted for private gain;

146 - Authorised officers who carry written authorizations and proof of identity, which they will produce on request, shall be admitted immediately to all parts of the premises at all reasonable times.

148 - a.The Licensee shall not permit an entertainment that involves special risks except with consent. b.The Licensee shall not permit any performances especially for children except with consent. c.The licensee shall not permit explosives or highly flammabl e substances to be brought onto the premises except with consent.

149 - a.The Licensee shall not permit the use of special effects, except with consent. b.The Licensee shall give to the Council at least 10 days notice in writing of any proposal to use special effects. The notice shall include, save in exceptional circumstanc es, exact details of the proposal including the date and time when the special effects can be demonstrated.

150 - Compressed or liquefied gases shall not be used except with consent. At least 10 days notice in writing shall be given to the Council of any proposal to bring storage cylinders into the premises

156 - The Licensee / Duty manager shall maintain a register indicating the numbers of staff, including any Door Supervisors and all performers, who are present when the public are present. This register shall be produced immediately on the request of an Authori sed Officer. This Condition does not apply to any premises that are being used for a closely seated audience.

158 - a.The Licensee / Duty Manager shall ensure that no nuisance is caused by noise emanating from the premises or by vibration transmitted through the structure of the premises. b.If required, legible notices shall be displayed at all exits requesting the p ublic to respect the needs of local residents and to leave the premises and area quietly.

160 - The Licensee shall ensure that all performances or activities minimize any danger to the public.

161 - The Licensee / Duty manager shall ensure that, whenever disabled people are present, adequate arrangements are made to enable their safe evacuation in the event of an emergency and that they are made aware of those arrangements.

170 - Refuse receptacles shall be emptied regularly.

171 - Access for emergency vehicles shall be kept clear and free from obstruction.

172 - a.The Licensee / Duty Manager shall ensure that an adequate and appropriate supply of first aid equipment and materials is available on the premises. b.If required, at least one suitably trained first aider shall be on duty when the public are present. I f more than one suitably trained first-aider is present, each person's responsibilities shall be clearly identified.

173 - a.Toilet accommodation shall be provided free of charge and be kept clean and in proper working order. b.An adequate supply of hot and cold (or warm) water, toilet paper in holders or dispensers, soap and suitable hand and face drying facilities shall b e provided in toilet accommodation.

176 - If required, a competent person shall be in charge of the electrical or other installation.

180 - a.The premises shall be effectively ventilated. b.Where the ventilation system is designed to maintain a positive air pressure within that part of the premises, that pressure shall be maintained whenever the public are present in that part of the premise s.

181 - a.Ventilation ducting and other shafts shall be kept clean. b.Any air filters shall be periodically cleaned or replaced so as to maintain a satisfactory air supply. c.All interior surfaces of extract ventilation ducting serving kitchens and serveries sh all be thoroughly cleaned as frequently as necessary to prevent the accumulation of grease and fat and at least once per year. d.Grease filters in extract ventilation hoods in kitchens and serveries shall be cleaned weekly or at other intervals as requir ed.

203 - The Licensee shall not permit conduct on the premises that is likely to cause disorder or a breach of the peace or drug misuse. In particular the licensee shall ensure that none of the following shall take place a) indecent behaviour, including sexual in tercourse, except as permitted by the Theatres Act 1968; b) the offer of any sexual or other indecent service for reward; c) acts of violence against person or property and / or the attempt or threat of such acts; and d) unlawful possession and / or su pply of drugs controlled by the Misuse of Drugs Act 1971.

288 - A CCTV system to be installed at the premises and be maintained in good working order (clear identifiable images) and be continually recording at all times the premises are in use under the licence. The CCTV system must be capable of capturing an image of every person who comes in to the premises. Signage indicating that CCTV will capture images of all persons entering the premises shall be displayed.

289 - All CCTV footage shall be kept for a period of 31 days and shall, upon request, be made available to officers of the police and council.

291 - On Thursdays, Fridays & Saturdays at least two SIA registered door supervisors will be employed at the premises from 9pm until all patrons have left the premises. They will be responsible for screening patrons entering the premises, conflict management, to maintain a safe accommodation limit and timely dispersal of patrons.”

293 - That staff employed in the supply of intoxicating liquor have completed a training scheme on relevant Licensing legislation

303 - That the licence holder shall require any promoter hiring the premises to complete the Metropolitan Police Venue Hire Agreement form and supply it to Police a minimum of 28 days prior to the date of hire. This must include events which are "private" events

307 - That the total number of persons that may be accommodated within the licensed area at any one time shall not exceed (one hundred and thirty) 130, the maximum that can be accommodated on the ground floor shall not exceed (one hundred) 100, and the maximum number that can be accommodated on the first floor should not exceed (thirty) 30.

309 - That all musicians and DJs must play electrically amplified music/audio/PA through a sound cut out device. The cut-out device shall be maintained at levels set to the satisfaction of the Council's Noise Team

310 - All exterior windows and doors, except exterior doors that have to be locked open for public safety, will remain closed during the operation of the entertainments licence.

311 - That notices shall be displayed and announcements made requesting that customers leave the premises in a quiet and orderly manner

312 - Customers shall use no outside area after midnight other than those customers who temporarily leave the premises to smoke a cigarette. Those who do temporarily leave for this purpose shall not be permitted to consume beverages whilst outside after this time.

313 - That there shall be no movement of musical or amplification equipment, to or from the premises, between 0000 and 0800 hours and the vehicle carrying such equipment shall be parked as far away from residential premises as reasonably practicable

336 - After 8pm on a Thursday, Friday & Saturday night a personal licence holder will be on the premises at all times the premises are in operation under the premises licence.

340 - Compliance with MPS "Safe & Sound" policy

485 - (1) The responsible person must ensure that staff on relevant premises do not carry out, arrange or participate in any irresponsible promotions in relation to the premises. (2) In this paragraph, an irresponsible promotion means any one or more of the following activities, or substantially similar activities, carried on for the purpose of encouraging the sale or supply of alcohol for consumption on the premises – (a) games or other activities which require or encourage, or are designed to require, encourage, individuals to - (i) drink a quantity of alcohol within a time limit (other than to drink alcohol sold or supplied on the premises before the cessation of the period in which the responsible person is authorised to sell or supply alcohol), or (ii) drink as much alcohol as possible (whether within a time limit or otherwise); (b) provision of unlimited or unspecified quantities of alcohol free or for a fixed or discounted fee to the public or to a group defined by a particular characteristic in a manner which carries a significant risk of undermining a licensing objective; (c) provision of free or discounted alcohol or any other thing as a prize to encourage or reward the purchase and consumption of alcohol over a period of 24 hours or less in a manner carries a significant risk of undermining a licensing objective; (d) selling or supplying alcohol in association with promotional poster or flyers on, or in the vicinity of, the premises which can reasonably be considered to condone, encourage or glamorise anti-social behaviour or to refer to the effects of drunkenness in any favourable manner; and (e) dispensing alcohol directly by one person into the mouth of another (other than where that other person is unable to drink without assistance by reason of disability).

487 - The responsible person must ensure that free potable water is provided on request to customers where it is reasonably available.

488 - (1) The premises licence holder or club premises certificate holder must ensure that an age verification policy is adopted in respect of the premises in relation to the sale or supply of alcohol. (2) The designated premises supervisor in relation to the premises licence must ensure that the supply of alcohol at the premises is carried on in accordance with the age verification policy. (3) The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and either (a) a holographic mark; or (b) an ultraviolet feature.

489 - The responsible person shall ensure that - (a) Where any of the following alcoholic drinks is sold or supplied for consumption on the premises (other than alcoholic drinks sold or supplied having been made up in advance ready for sale or supply in a securely closed container) it is available to customers in the following measures - (i) Beer or cider: 1/2 pint; (ii) Gin, rum, vodka or whisky: 25 ml or 35 ml; and (iii) Still wine in a glass: 125 ml; (b) these measures are displayed in a menu, price list or other printed material which is available to customers on the premises; and (c) where a customer does not in relation to a sale of alcohol specify the quantity of alcohol to be sold, the customer is made aware that these measures are available,

491 - 1. A relevant person shall ensure that no alcohol is sold or supplied for consumption on or off the premises for a price which is less than the permitted price. 2. For the purpose of the condition set out in paragraph (1): (a) “duty” is to be construed in accordance with the Alcoholic Liquor Duties Act 1979; (b) "permitted price" is the price found by applying the formula P = D + (D x V), where- (i) P is the permitted price, (ii) D is the amount of duty chargeable in relation to the alcohol as if the duty were charged on the date of the sale or supply of the alcohol, and (iii) V is the rate of value added tax chargeable in relation to the alcohol as if the value added tax were charged on the date of the sale or supply of the alcohol; (c) "relevant person" means, in relation to premises in respect of which there is in force a premises licence – (i) the holder of the premises licence: (ii) the designated premises supervisor (if any) in respect of such a licence; or (iii) the personal licence holder who makes or authorises a supply of alcohol under such a licence; (iv) "relevant person" means, in relation to premises in respect of which there is in force a club premises certificate, any member or officer of the club present on the premises in a capacity which enables the member or officer to prevent the supply in question; and (v)"value added tax" means value added tax charged in accordance with the Value Added Tax Act 1994. 3. Where the permitted price given by paragraph (b) of paragraph 2 would (apart from this paragraph) not be a whole number of pennies, the price given by that sub-paragraph shall be taken to be the price actually given by that sub-paragraph rounded up to the nearest penny. 4. (1) Sub-paragraph (2) applies where the permitted price given by paragraph (b) of paragraph 2 on a day ("the first day") would be different from the permitted price on the next day ("the second day") as a result of a change to the rate of duty or value added tax; (2) the permitted price which would apply on the first day applies to sales or supplies of alcohol which take place before the expiry of the period of 14 days beginning on the second day.

793 - That all staff employed in the supply of intoxicating liquor complete a refresher training course on the relevant licensing legislation.

808 - That there be no entry or re-entry to the premises after 00.30hrs Monday to Sunday with the exception of smokers.

840 - That there be a change in the senior management team of the day to day operation of the Blue Eyed Maid.

841 - That the operating hours be reduced on Fridays and Saturdays to 11.00hrs to 02.30hrs and all licensable activities to cease at 02.00hrs.

843 - That an incident log shall be maintained and details of all refusals recorded. This book shall be reviewed monthly by the DPS and any actions taken shall be recorded in the book and be signed off by the DPS. This log shall be maintained on the premises and shall be made available for inspection by authorised officers.

844 - That a dispersal policy be introduced to provide quick dispersal of customers from outside the premises after closing time.
