Conditions: 100 - No supply of alcohol may be made under the Premises Licence - (a). At a time when there is no Designated Premises Supervisor in respect of the Premises Licence; or (b). At a time when the Designated Premises Supervisor does not hold a Personal Licence or his Personal Licence is suspended.

101 - Every supply of alcohol under the Premises Licence must be made, or authorised by, a person who holds a Personal Licence.

102 - The admission of children to films given under this licence must be restricted in accordance with the recommendations of the British Board of Film Classification or of the licensing authority itself.

152 - There will be a duty manager available at all times while the premises is open to the public

172 - Floors and traffic routes within the venue shall be kept free from obstacles, obstructions and any other article or substance that may cause persons to slip, trip or fall

173 - That all installations shall be protected by a residual current device RCD set 30mA current trip capacity or lower

239 - All relevant external doors and windows, other than the main front door, shall remain closed during licensed entertainment other than for emergency access and egress, and they shall be marked as such during any performance licensed entertainment.

265 - That all cables run between the mains supply and the equipment shall be kept as short as possible. Where applicable cables shall be secured and covered to prevent slips, trips and falls

270 - That a CCTV system be installed at the premises and be maintained in good working order and be continually recording at all times the premises are in use under the licence. The CCTV system must be capable of capturing an image of every person who enters the premises

286 - That all staff are trained in their responsibilities under the licensing act 2003 and training records to be kept and updated every 6 months and shall, upon request, be made immediately available to Officers of the Police and the Council

288 - That any regular and external promoters hiring the premises to complete the 'Venue Hire Agreement’ and once completed, you shall ensure that a copy of the agreement is provided to the Police and Licensing Unit a minimum fourteen days prior to the date of hire

289 - That no recorded music be played for the sole purpose of the paying audience and public to dance to

293 - Any external cleaning of the premises shall occur between 08.00 -20.00hrs Mon - Sat and not at all on Suns and Bank Holidays.

303 - Suitable notices shall be displayed and announcements made requesting people to leave the premises in a quiet and orderly manner so as not to disturb local residents

311 - All deliveries and collections shall occur between 08.00 -20.00hrs Mon - Sat, 09.00 - 18.00hrs Suns and not at all on Bank Holidays.

316 - There shall be only acoustic, unamplified music, speech and song, with no loud performances including drumming or brass instruments, in the Tiny Theatre and Lobby Bar areas of the premises.

326 - All staff trained in Challenge 21 policy and notices displayed to that effect

336 - That all CCTV footage shall be kept for a period of thirty one (31) days and shall, upon request, be made available to officers of the Police and the Council to inspect.

340 - A risk assessment for each performance will be undertaken by a competent person to identify foreseeable hazards associated with each performance

341 - Arrangements will be made for the effective planning, control and monitoring of each performance

342 - Arrangements will include procedures for dealing with any foreseeable emergency that may arise and detail steps to be taken should all or any part of the venue need to be evacuated

343 - All arrangements will be put in writing and shall be made available at the request of the council

344 - Any necessary contacts with external services, will be arranged particularly as regards first aid, emergency medical care and rescue work

345 - There will be a minimum of 1 steward per 100 audience members or part thereof available at all times during the performances

346 - Stewards will be easily identifiable by means of a clearly displayed identity badge

347 - Prior to each performance all persons connected with the performance will be briefed as to the findings of the risk assessment. They will be briefed about all preventative and protective measures in place and details of the emergency evacuation procedures

348 - Stewards will remain with the audience at each exit to the performance space

349 - That not more than four hundred and sixty (460) persons in addition to theatre personnel shall be admitted into the venue at any one time

350 - That admission to the performance shall be by ticket only

351 - All areas of the venue to be used for performances will be of a stability and solidity that is suitable for the intended use. That all equipment, devices and systems used in the performance shall be maintained in a good state of repair and effective working order

352 - All areas of the venue will be provided with sufficient lighting

353 - In the event of failure of the main lighting, emergency lighting shall come on to allow persons to leave the venue in a safe and orderly manner

354 - That no persons will be permitted to return to the venue until the mains lighting has been restored and the emergency lighting batteries are fully recharged

355 - All areas of the venue will be kept in a clean and tidy condition

356 - Waste material shall not be permitted to accumulate in the venue unless stored in an appropriate receptacle prior to proper collection and disposal

357 - That all fire alarm systems must be operational at all times and in all areas of the building

358 - Fire fighting equipment shall be provided at positions identified in the risk assessment as being at risk of fire

359 - All fire fighting equipment will be placed in readily accessible positions and the location clearly signposted

360 - A first aid kit will be kept complete and in an accessible location at all times

361 - Emergency routes and exits shall be checked prior to each performance to ensure no obstacles or obstructions are present

362 - Appropriate permanent toilet accommodation will be provided

363 - That any temporary electrical installations shall be undertaken by a suitably qualified and competent person

364 - Seating will be secured together in lengths not less than 4 and in rows of no more than 12 with a good clear passage between each row and with steps and layout conforming to standard terms and conditions relating to performance

365 - All Southwark Playhouse staff are CRB checked

366 - All disused equipment including portable LPG gas cylinders to be removed from the premises

367 - A personal licence holder will be on the premises at all times intoxicating liquor is being offered for sale

368 - The maximum noise level within the Little and Large Theatres shall not exceed 90dB(A) L90 5min

369 - Noise outbreak to the rear of the theatre building shall be monitored at the curtilage of the nearest residential premises for inaudibility at a) the first performance of a new production, or b) the first time an activity type is tried in any of the 4 performance spaces. This is to ensure internal noise levels are appropriate for the space/s in which they are being performed.

370 - All inaudibility noise monitoring information shall be kept in a log which annotates the date, time, performance space being used, production or activity, any remedial actions to reduce noise intrusion, and, signatures from the person undertaking the monitoring/remedial actions.

371 - All records of external noise monitoring shall be kept at the premises for immediate inspection by Council and Police Officers on request.

372 - The main front doors shall be fitted with self closers and acoustic seals.

373 - Before licensed entertainment is permitted to take place, the licensee shall submit details of a ventilation system with acoustic treatment for the inflow and outflow of air for the premises, for approval by the EP Team. The approved ventilation system shall then be installed and function correctly prior to the commencement of any licensed entertainment.

374 - Patrons shall not take drinks outside the premises.

375 - The maximum number of people permitted to be outside the building at the front of the premises will not exceed 10 at any one time after 22.00hrs.

376 - There will be clearly visible signs requesting people to leave the premises quietly at all entrances and exits.

377 - All staff shall be trained with regard to the noise control requirements of the premises license.

378 - All records of staff training with regard to the noise control requirements of the premises license shall be kept at the premises for immediate inspection by Council and Police Officers on request.

379 - That all fire fighting equipment shall be maintained in efficient order and shall be covered by a current certificate of inspection

380 - That during the performance a person trained in the effective use of fire fighting equipment shall be on hand at all times

485 - (1) The responsible person must ensure that staff on relevant premises do not carry out, arrange or participate in any irresponsible promotions in relation to the premises. (2) In this paragraph, an irresponsible promotion means any one or more of the following activities, or substantially similar activities, carried on for the purpose of encouraging the sale or supply of alcohol for consumption on the premises – (a) games or other activities which require or encourage, or are designed to require, encourage, individuals to - (i) drink a quantity of alcohol within a time limit (other than to drink alcohol sold or supplied on the premises before the cessation of the period in which the responsible person is authorised to sell or supply alcohol), or (ii) drink as much alcohol as possible (whether within a time limit or otherwise); (b) provision of unlimited or unspecified quantities of alcohol free or for a fixed or discounted fee to the public or to a group defined by a particular characteristic in a manner which carries a significant risk of undermining a licensing objective; (c) provision of free or discounted alcohol or any other thing as a prize to encourage or reward the purchase and consumption of alcohol over a period of 24 hours or less in a manner carries a significant risk of undermining a licensing objective; (d) selling or supplying alcohol in association with promotional poster or flyers on, or in the vicinity of, the premises which can reasonably be considered to condone, encourage or glamorise anti-social behaviour or to refer to the effects of drunkenness in any favourable manner; and (e) dispensing alcohol directly by one person into the mouth of another (other than where that other person is unable to drink without assistance by reason of disability).

487 - The responsible person must ensure that free potable water is provided on request to customers where it is reasonably available.

488 - (1) The premises licence holder or club premises certificate holder must ensure that an age verification policy is adopted in respect of the premises in relation to the sale or supply of alcohol. (2) The designated premises supervisor in relation to the premises licence must ensure that the supply of alcohol at the premises is carried on in accordance with the age verification policy. (3) The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and either (a) a holographic mark; or (b) an ultraviolet feature.

489 - The responsible person shall ensure that - (a) Where any of the following alcoholic drinks is sold or supplied for consumption on the premises (other than alcoholic drinks sold or supplied having been made up in advance ready for sale or supply in a securely closed container) it is available to customers in the following measures - (i) Beer or cider: 1/2 pint; (ii) Gin, rum, vodka or whisky: 25 ml or 35 ml; and (iii) Still wine in a glass: 125 ml; (b) these measures are displayed in a menu, price list or other printed material which is available to customers on the premises; and (c) where a customer does not in relation to a sale of alcohol specify the quantity of alcohol to be sold, the customer is made aware that these measures are available,

491 - 1. A relevant person shall ensure that no alcohol is sold or supplied for consumption on or off the premises for a price which is less than the permitted price. 2. For the purpose of the condition set out in paragraph (1): (a) “duty” is to be construed in accordance with the Alcoholic Liquor Duties Act 1979; (b) "permitted price" is the price found by applying the formula P = D + (D x V), where- (i) P is the permitted price, (ii) D is the amount of duty chargeable in relation to the alcohol as if the duty were charged on the date of the sale or supply of the alcohol, and (iii) V is the rate of value added tax chargeable in relation to the alcohol as if the value added tax were charged on the date of the sale or supply of the alcohol; (c) "relevant person" means, in relation to premises in respect of which there is in force a premises licence – (i) the holder of the premises licence: (ii) the designated premises supervisor (if any) in respect of such a licence; or (iii) the personal licence holder who makes or authorises a supply of alcohol under such a licence; (iv) "relevant person" means, in relation to premises in respect of which there is in force a club premises certificate, any member or officer of the club present on the premises in a capacity which enables the member or officer to prevent the supply in question; and (v)"value added tax" means value added tax charged in accordance with the Value Added Tax Act 1994. 3. Where the permitted price given by paragraph (b) of paragraph 2 would (apart from this paragraph) not be a whole number of pennies, the price given by that sub-paragraph shall be taken to be the price actually given by that sub-paragraph rounded up to the nearest penny. 4. (1) Sub-paragraph (2) applies where the permitted price given by paragraph (b) of paragraph 2 on a day ("the first day") would be different from the permitted price on the next day ("the second day") as a result of a change to the rate of duty or value added tax; (2) the permitted price which would apply on the first day applies to sales or supplies of alcohol which take place before the expiry of the period of 14 days beginning on the second day.

4AL - The findings of the risk assessment will be put in writing and shall be made available at the request of the council
